Better Dis-stress tolerance through accepts

Distress is everywhere, no matter how many times you wish you could run and hide from it. But instead of avoiding distress, you can learn to better tolerate it using a formula called ACCEPTS.

ACCEPTS is one of the methods of distress tolerance taught as part of Dialectical Behavior Therapy (DBT). ACCEPTS stands for Activities, Contribute, Comparisons, Emotions, Push away, Thoughts, and Sensations.

How does ACCEPTS work? Let’s say you just had an interview with an employer for a new job that you would really like. At the end of the interview, the employer says you will hear back about whether or not you’re hired in three days. Three days can be a very long time not to hear about such an important and possibly life-changing decision.

But instead of wringing your hands for three days, you can utilize ACCEPTS and change your internal state to a positive one. Here are some ways you can use ACCEPTS and Dialectical Behavior Therapy to better manage your internal distress:

Activities: Participating in an activity you enjoy will not only distract you from whatever is bothering you, but it can instill a sense of calm. Find whatever activity it is that you enjoy and do it as often as possible. If you really like reading magazines, go to your local library and read some for free. If you really like being outdoors, go for a hike or a walk around the neighborhood.

Contribute: While you’re waiting, help someone in your family cook dinner or volunteer your time by picking up trash at the park. Helping others and contributing to society makes you feel good about yourself and keeps your mind focused on more productive things.

Comparisons: Remember when you could not even get a job interview? Remember when you didn’t have the skills to obtain this job? Compare your current state to a past state in which you were not doing as well so you can see how far you’ve come.

Emotions: If your emotions are feeling wired, watch or read something comical, or simply watch children playing in the park. Anything that uplifts your spirits will help you to cope.

Push away: There is nothing you can do about the employment decision, so it doesn’t make any sense to think more about it. Push it out of your mind and occupy yourself with something more pressing, like paying bills or taking care of odd things that need to be done around the house.

Thoughts: Still thinking about the job? Stop! Sing a song. Count down to zero from 100. Say a mantra or prayer. Try to think of a state or country for each letter of the alphabet. The mind is capable of thinking so many thoughts, but only you can choose which ones those will be.

Sensations: Nothing takes the mind off something more than a prominent physical sensation. Have some sour candy or take a cold shower. Sometimes just feeling the sunlight on your face does the trick.

Using these DBT distress tolerance techniques anytime you are facing a stressful situation or one that seems unmanageable can help you to focus on the present and stop worrying about what you can’t control.

Accepting crisis using DBT

Accepting crisis might be a bit of a frightening proposition. None of us invites crisis to visit us, and yet every one of us will have crisis as part of our lives. Crisis can often make you feel out of control, especially if you experience emotions more intensely than most. If you also have the feeling that you cannot handle what you are going through, it can make things really difficult.

Crisis is defined by the individual, and can be triggered by huge things such as a death in the family, loss of a relationship, or loss of a pregnancy. It can also be triggered by small things such as not liking your hair one day or realizing you planned two things to happen at one time. Whatever the cause, large or small, it is necessary to be skillful in order to get through crisis.

I recently had to give a presentation at a hospital. I had a lot of anxiety about the proposition. I would be developing the presentation specifically for hospital staff working with people with Borderline Personality Disorder. I also had all of my normal responsibilities. I have household chores, two small children, a wife in nursing school, and two jobs. Without careful planning these things mount up and create crisis. So, I got a panicked feeling, noticed anxiety like a little electricity in my chest, tightness in my shoulders, and a sense that I could not do this!

The key to handling crisis is to get through it without making it worse. When we feel emotionally overwhelmed, especially if the emotion is really intense, we are more likely to do all sorts of things we will regret later. We may drink, use drugs, cut, attempt suicide, have risky sex, run away, or spend lots of money. All of these things feel good immediately, but have larger consequences down the road. So, how do we handle these crisis moments?

Handling Crises through ACCEPTS

We can handle crises through the acronym ACCEPTS, which is taught in Dialectical Behavior Therapy (DBT). Activities can help briefly distract us from the crisis and center our mind a little more. Contributing to something bigger than yourself can give perspective. It does not have to be formal volunteering, maybe just get someone a glass of water in your house.

Comparisons can help by comparing yourself to a time when you were doing better and realize that you will not always be in crisis. Do things that evoke other Emotions. Read a funny book, watch a scary movie, listen to calming music. Push the crisis away for a little bit. The crucial part here is a “little bit.” Push away too long and we get avoid, and that generally what creates crisis in the first place!

Use other Thoughts, any thoughts. Count silverware, do math in your head, anything to put another thought in there. And use Sensations. Cold works especially well to shock you back into the moment. Cold to the face has a physical effect on the brain at the emotion center. Crazy, but true. You can also use sour candy, tangy fruit, or a wall (sit until your legs burn like crazy).

The title of this article is “Accepting Crisis” because we cannot do anything about a situation until we accept that we are in it. I put off preparing for my presentation about a million times, approximately. When we cannot change a situation, we need to accept (or acknowledge) that situation.

Focusing on breathing is a great way to acknowledge the moment. Concentrate on your breath all the way in to your abdomen and all the way back out.

Become aware of where you are. I am sitting in a chair. I feel where my back and buttocks are hitting the chair. I feel the smoothness of the keys as I type. I see the letters form on the screen. Become aware of every little nuance.

What skills will I learn in Dialectical therapy

Learning new skills is critical in Dialectical Behavior Therapy (DBT), and is a focus of every individual therapy session, skills group, and phone coaching session.

In DBT, you will develop four important skills: mindfulness, emotion regulation, interpersonal effectiveness, and distress tolerance.

Mindfulness: Mindfulness is a core principle of Dialectical Behavior Therapy. Being mindful helps you control your focus so you can stop worrying about the future or obsessing about the past.

Through the DBT skill of mindfulness, you will learn to recognize and release your thoughts by feeling the “present moment” so that you can learn more about what makes you feel good or feel bad. As you release your thoughts, you focus on skills like observing your environment within yourself or outside of yourself.

If you choose to develop the skill of mindfulness through meditation, you will learn to focus your mind and increase your attentiveness through a frequent meditation practice.

In developing your mindfulness skills, you will learn to describe what you observe without judgment, and without labeling thoughts. Instead, you will simply experience being present and connecting with your sense of self.

Emotion Regulation: Emotion regulation means that you understand what your emotions are, how they function in your life, and how to experience those emotions in a way that is not overwhelming.

One practice you might work to develop is to “commit to your opposite action,” by realizing that every emotion has an action, and by choosing the opposite action (to go to a party, for instance, when you normally feel nervous about meeting new people and would opt to stay home). Practicing this skill helps you see that emotions can be managed.

Interpersonal Effectiveness: This DBT skill involves starting new relationships, improving existing relationships, or ending bad relationships.

To develop this skill, you may work on smaller steps like describing a situation, expressing yourself, stating your position clearly, reinforcing your position, and staying mindful. Over time, you will feel your relationships and interactions with others improve as you either change something or resist changes someone is trying to impose on you.

Distress Tolerance: The goal with this DBT skill is to tolerate emotional pain without resorting to self-harm or self-destructive behaviors.

According to Dr. Marsha Linehan, the founder of Dialectical Behavior Therapy, the ability to tolerate and accept distress is an essential mental health goal because pain and distress are a part of life. Moreover, distress tolerance is part of any attempt to change oneself.

When you begin DBT, you will learn techniques for distress tolerance, such as distraction (through activities, sensations, or thoughts), self-soothing, and other methods, such as prayer or relaxation.

A true Dialectical Behavior Therapy program will incorporate these four DBT skills throughout treatment to help you change with acceptance to live a life worth living.
